Where do I find current BSA Medical Form?

Created by Dawn Niece, Modified on Tue, Apr 2 at 1:43 PM by Chris Guarniere

Every few years, the BSA updates its Annual Health and Medical Record after consulting with health care professionals, Scout executives, council and camp health officers and other experts to ensure it is up-to-date and helpful.

The only way to assure you have the proper documents is to access them from this website:

www.scouting.org/health-and-safety/ahmr. This is the only source for the Boy Scouts of America’s AHMR.

NOTE:  Summer Camps operated by the Greate New York Councils require additional information beyond the standard BSA AHMR.  Please consult the camp website your unit registration contact to ensure you have the correct form for the activity that you plan to participate.
